Explanation of Journal Notes


The purpose of the Journal Notes form is to provide a common format for the employee and supervisor to maintain notes throughout the evaluation period that will assist at the time of the performance evaluation meeting.  The supervisor will use the Journal Notes form to identify examples of strengths and areas for improvement noted during the employee’s evaluation period. In addition, Journal Notes allow for brief descriptions of special tasks assigned, documentation of corrective actions, and recollections of positive events. Each employee should use the Journal Notes form to record their own specific accomplishments.
