Explanation of Co-worker Evaluation

 


Co-worker Evaluations are used to assist supervisors in obtaining feedback on the employee’s job performance. The supervisor seeks peer feedback for the evaluation by requesting that a co-worker complete the Co-worker Evaluation form or interviewing co-workers in person or over the telephone to gather answers to the questions on the form. ALL INFORMATION MUST REMAIN CONFIDENTIAL.

The attached form should be used to obtain feedback from the staff member’s co-workers. The Co-Worker Evaluation form may be copied and routed by the supervisor of the employee being evaluated to individuals who have a working relationship with the employee’s department.

It is recommended that no more than 5 peer contacts be made to gather information and assess the employee’s performance. If the obtained feedback is not deemed to have provided adequate information, or the area of responsibility of the employee being evaluated is highly critical, then additional feedback contacts should be made. The co-worker evaluation is intended to provide sufficient feedback to determine if any areas of the employee’s job performance need improvement. The discussion of the employee’s performance should remain focused on the five questions identified on the Co-worker Evaluation form.
