Tutoring for Success applies to any student whose overall performance in the course falls below 75%. The instructor will create the task in the Student Engagement Portal (Watermark) to direct the student to the appropriate tutoring service, which may be faculty- or SI-led, discipline-specific, and/or general. The tutoring service assigned, the due date for when the tutoring must be completed, and the amount of tutoring required are at the discretion of the instructor. Additionally, the task will alert the student’s success team. Students who do not fulfill the assigned tutoring task may be subject to program- and course-specific penalties that could result in a grade reduction and/or in not being allowed to progress in the course until the tutoring requirement has been satisfied.
Students who do not attend class on or prior to the census date will be administratively dropped. Effective Fall, 2016
Students who wish to withdraw from a course must complete all steps indicated on the Academic Withdrawal Request form by the course withdrawal deadline.
NOTE: Students who are attending Texas institutions of higher education, for the first time fall 2007 and later, may not withdraw from more than six courses during their academic career. This withdrawal limitation does not include dual credit or developmental classes (Senate Bill 1231 Rule 4.10.) For more information on Drop and Withdrawal Policies, please visit the Registrar's Office Web site.

GRADING CRITERIA & INSTRUCTIONS:
All paperwork, journals, CPEC, and other assignments are due by 1700 in the corresponding area on Thursday following your Wednesday clinical rotation. (You may drop off your hard copies on Wednesday after your clinical day ends if you'd like).
Please know the main office where you will drop your paperwork off closes promptly at 1700 on Thursdays! There will be no exceptions for missing the due date or time, and we will not accept any paperwork slid under the office door; it must be in my box upside down for privacy reasons.
The entire assignment (hard copies and online journals) must be submitted by the proper time in the appropriate area to get credit for the whole week. If a partial is submitted late or nothing is submitted, the grade for the week is a zero. No makeup work is accepted.
1. The Clinical Preceptor Agreement must be signed by the nurse you worked with and turned in for the ****first two weeks only****. If the nurse informs you they have already filled it out or refuses, do not force them. Please write a note on the form and turn it in with the other forms due that week. *Due first two weeks only*
2. The Student Evaluation Form and Evaluation of Clinical Preceptor are due in the instructor’s box by the afternoon following each rotation. *Due Weekly*
3. Online journal assignments are due weekly and must be placed in the specified drop-box on Blackboard Online by 1700 the day following each rotation. If you place them in the incorrect online journal dropbox, it is a zero for that week. Please check the dates! *Due Weekly*
If all assignments are not completed or turned in by 1700 on Thursday, the student will receive a zero for the entire week. The papers and journal assignments are entered as one grade.
ATTENDANCE POLICY
The clinical schedule will be handed out during clinical orientation. For this course, the clinical is on Wednesdays. You will be at clinics for a full 8 hours. Please do not schedule any events during this time frame. Absence of any part of the clinical day will result in a full 8-hour clinical make-up.
Regular and ON-TIME attendance is necessary for satisfactory achievement.
The absence of any part of the clinical day will result in a full 8 hr clinical makeup on a medical-surgical rotation.
Regular attendance is necessary. The student must have 100% clinical attendance. All clinical absences are cumulative for the semester and must be made up. The max days that can be made up are two from ALL clinical courses in level two—missing more than two days in ANY or ONE clinical rotation results in failure of the clinical rotation.
Calling out sick:
If you are sick before the clinical day, email the instructor at least two hours before you report. You must also contact the assigned site to let them know you will not be attending once they are open. There will be no assignments or paperwork due, and you will make the day up on the Thursday/Friday medical surgical make-up days.
IMPORTANT INFORMATION
If you, for any time, must leave during your clinical shift for personal reasons, approval from the instructor is mandatory. Leaving the facility/site at any time without an instructor's permission will result in a class three clinical warning or dismissal from the program. In addition, any time absent from the day will count as a full clinical absence. This is considered abandonment of patients and is against the Texas Board of Nursing rules.
******If released from the site early, you must text or call the instructor BEFORE leaving the facility parking lot for approval****. Due to Board of Nursing time requirements, your instructor may have you report to campus for the remainder of your eight hours.
CLINICAL SCHEDULE
Students will receive clinical schedules after the course is in progress. Clinicals will be on Wednesdays at various medical facilities. Times will vary depending on the clinical site.
