\ Access to computer and printer (with cartridges and paper), transportation, 3 ring binder, dividers and pen.
The student is to successfully complete:
1. A 256 hour placement in an approved health care agency whose primary function is the counseling and treatment of chemically addicted individuals is required.
2. Meet as a seminar group four times during the semester (January 13th, February 24th, March 30th, and May 4th, 2012) in Durrett 201, for 60 minutes each (11:30 a.m. – 12:30 p.m.)
3. A contract signed and completed by agency. This should be in the logbook and a copy turned in at the second meeting.
4. A journal which includes a log of hours and days worked at the assigned agency. This record is kept on the KSA form and is to be signed off weekly by the site supervisor. In addition, the student is to keep a daily log so they can accurately write a complete report in their weekly and monthly summaries. The on site supervisor is required to sign off on the weekly summary. Forms are available online at http://www.actx.edu/substance.
5. A journal in a 3-ring binder with labeled dividers. Suggested titles include:
6. Submit in writing (typed) two reaction papers (one at 125 hours and one at 256 hours) about the learning taking place and your growth during the experience.
7. Have completed by field supervisor and returned to the instructor a mid-term and final evaluation.
8. If for any reason evaluations, papers or journals are late, 20% of points for each item will be automatically deducted.
