Assess various patient needs; formulate a dental hygiee care plan for the medically and/or dentally compromised patient; and describe supplemental instrumentation techniques.

Instructor Objectives for the Course

Chapter objectives will coordinate with the materials presented in lecture, handouts, reading assignments and exams.  The student will know that these objectives have been successfully completed if he or she earns a final course grade of “C” or higher as evaluated by an instructor.  The standard for the objectives may be one or any combination of the following:  a.  As given in lecture, b.  As demonstrated and/or discussed in clinic/lab, c.  As given in handouts and d.  As stated in course text:

Chapters 1-6,43

  1. Discuss cardiovascular disease, including:
  • Discuss cardiovascular disease risk factors.
  • Critically evaluate the relationship between cardiovascular disease and periodontal disease.
  • Identify signs and symptoms of rheumatic heart disease, infective endocarditis, valvular heart defects, hypertension, coronary heart disease, cardiac arrhythmias, congestive heart failure, and congenital disease.
  1. Identify the types of cardiovascular surgery.
  2. Discuss oral manifestations of cardiovascular medications.
  3. Discuss the prevention and management of cardiac emergencies, including:
  • Determine the need for emergency medical care in clients with coronary heart disease
  • Develop a dental hygiene diagnosis and care plan for a client with cardiovascular disease.

Chapter 20

  1. Discuss the connection between periodontal infections and overall general health, including why the presence of bleeding upon periodontal probing means that the “door is open” for a wide range of adverse effects on the overall health of an individual.
  2. Explain the potential connection between periodontal diseases and coronary heart disease (atherosclerosis, myocardial infarction), cerebrovascular disease (stroke), pregnancy complications and adverse outcomes, diabetes mellitus, pulmonary diseases, neurologic diseases, gastrointestinal diseases, and cancer of the stomach and pancreas.

  1. Discuss basic concepts of nonsurgical periodontal therapy including:  a.  Explain similarities and differences between disease activity and disease severity, b.  Differentiate among nonsurgical periodontal therapy, oral prophylaxis, and periodontal maintenance therapy.

Identify other specific types of diabetes mellitus.
  3. Recognize the pathophysiology of diabetes, including the signs, symptoms, and oral and systemic complications.
  4. Recognize a diabetic emergency, and take appropriate action for management.

  2. Discuss pathophysiology of autoimmune diseases.
  3. Describe pharmacologic considerations for autoimmune diseases.
  4. Discuss how autoimmune diseases affect the dental hygiene process of care including:  a.  Recognize the systemic and oral manifestations of common autoimmune diseases covered in this chapter, b.  Identify human needs related to each of the autoimmune diseases listed in this chapter and describe their implications for dental hygiene care. c.  Develop a dental hygiene care plan appropriate for persons with autoimmune diseases.

Chapter 49

  1. Define solid organ transplant and the United Network for Organ Sharing.
  2. Discuss solid organ transplant candidates, including:  a.  Determine oral health needs of solid organ transplant candidates, including realistic expectations for persons living with end-stage organ disease, b.  Manage xerostomia as a special consideration in persons living on dialysis, c.  Develop a dental hygiene care plan for the solid organ transplant candidate, d.  Interact with members of the pretransplant and post-transplant healthcare teams.
  3. Discuss dental care after solid organ transplantation, including:  a.  Determine oral health needs of solid organ transplant recipients, b.  Identify the actions indicated immediately after transplant, c.  Develop a dental hygiene care plan for the solid organ transplant recipient.

Chapter 50

  1. Discuss respiratory disease including:  a.  Identify the risk factors, signs and symptoms, related medications, and dental hygiene care implications for the following respiratory diseases:  asthma, chronic obstructive pulmonary disease (chronic bronchitis and emphysema), and tuberculosis.
  2. Develop a dental hygiene care plan for a person with a respiratory disease.

Instrumentation Objectives

  1. Discuss general morphologic considerations, including
  • Describe the roots of each of the permanent teeth in terms of numbers, shapes, and characteristic landmarks.
  • Discuss significance of root morphology and positioning in the alveolar bone to root instrumentation.
  • Describe variations in root structure that may affect the dental hygiene process of care.
  • Explain how the contour of the cementoenamel junction, root morphology in both horizontal and vertical directions, furcation location, and root concavities influence instrument adaptation on root surfaces.
  • State the importance of tooth alignment to instrument adaptation on root surfaces.
  1. Identify variations of root form and explain the significance for root instrumentation.
  2. Describe the principles of instrumentation of the Gracey curette.

Research

  1. Write a research paper in APA format on a “special needs” topic.
  2. Develop a PowerPoint from the research paper.
  3. Present an oral presentation  from the research paper in front of peers and faculty.

Expected Student Behavior

 A high standard of conduct is expected of all Amarillo College students.  The Student Code of Conduct is defined in the Amarillo College Student Rights and Responsibilities publication.  The document may be located at www.actx.edu under the student tab.

"Amarillo College expects a high standard of conduct from its students.  Students are expected to comply with the law, respect proper constitutional authority, and obey College policies, rules and regulations.  Unacceptable conduct is defined as an action or behavior that is contrary to the best interest of a fellow student or students, the faculty or staff, the College, or the teaching/learning process.  Students failing to perform according to established standards may be subject to disciplinary action.  Policies, standards, rules and regulations apply to students attending College activities whether the activity takes place on or off campus.  Each student shall be responsible for becoming knowledgeable of the contents and provisions of College policies, rules, and regulations concerning student conduct.  In some cases, individual programs may have approved policies for student conduct which also apply to student conduct in those programs.  Integrity and common sense will guide the actions of each member of the college community both in and out of the classroom.  Any student who fails to perform according to expected standards may be disciplined."

The Amarillo College Dental Hygiene Program has specific policies which apply to student conduct within the program.  These policies may be implied or written.   The course syllabus and the Program Manual are sources for specific student conduct policies for dental hygiene students at Amarillo College.

Professional Standards

  • All dental hygiene students are expected to demonstrate professionalism in behavior, manner, and judgment in the dental hygiene clinic and clinic rotation sites.  The are of professionalism includes appearance, asepsis, patient management, peer, faculty and staff interaction and documentation.  Guidelines for professionalism and professional appearances are discussed in the AC Dental Hygiene Clinic Manual.  Professionalism will be evaluated on a continual basis.
  • Students will exhibit professional maturity and an acceptable level of clinical judgment as determined by faculty.  Unprofessional behavior will be documented in written form by the faculty.  Unprofessional behavior may be defined as a continual pattern of misconduct and/or an egregious act that endangers the health of parents, students, faculty, and/or College.  Egregious acts also include violations of the Texas Dental Hygiene Practice Act and falsification of patient records and/or faculty signatures.
  • Students must be competent in the application of the principles of ethical reasoning, ethical decision making and professional responsibility as they pertain to patient care.  Dental hygienists should understand and practice ethical behavior consistent with the professional code of ethics throughout their educational experiences.  The following are examples which are considered professional standards.  There are representative examples and may not be all inclusive.  For maximum learning to occur, the student is expected to demonstrate professional conduct and judgment at all times.
  • Student is concerned with excellence in learning rather than just meeting minimal criteria.
  • Student maintains his/her composure, dealing with conflict in a constructive way.
  • Student exhibits an attitude of respect for classmates, faculty and staff.  If a student has a conflict with a grade given by the instructor, the student must follow the outlined grievance procedure.  Faculty will not change a grade given to a student from another faculty member.

Instructor Help and Student Participation

Amarillo College Dental Hygiene Mentoring Program

The student has a very important role in the mentoring process.  Each student will be assigned a mentor at the beginning of the semester.  The mentor/mentee list will be posted in the locker room and emailed to each student.  As partners in the mentoring relationship, students have responsibilities to:

  • Contact mentors and arrange for a mentoring appointment – 1 in February and 1 in April
  • Be aware of their mentoring needs and discuss these needs with mentors
  • Maintain regular communication with their assigned mentors

Prepare for mentoring meetings by:

  • Setting daily and weekly goals for program progression
  • Assessing attainment of goals
  • Identifying impediments to achieving goals

It is the student’s responsibility to contact mentors and arrange appointments.  Failure to arrange and/or keep appointments will result in a two-point deduction from the final clinical course grade for each occurrence in the semester.

Mandatory Skills Building/Tutoring

In an effort to increase student success in DHYG courses, and to assist students having difficulty understanding complex subject areas, any student who scores below a 75% on any exam (excluding the final exam) must complete a mandatory skills building assignment before taking the next exam.  Before receiving a skills assignment, you must first schedule a time with the instructor.  The assignment will be given at that time.

 

 

 

 

Electronic and Recording Devices

In order for maxim learning to occur, all electronic recording devices and cell phones are prohibited in this course.  If a student is caught with a cell phone in hand or in a uniform pocket, or actively texting, talking or surfing the internet during class, the student will be dismissed and will receive a grade of "0".

Each student is assigned a mailbox located inside the student dressing room.  Students are required to check his/her mailbox daily for various communication from faculty.

First and second year dental hygiene students have designated areas in a common bulletin board in the student dressing room where various announcements may be posted.

Academic Grievance Procedure

The course will consist of chapter exams, a mid-term exam,  outside assignments utilizing the following website:   www.dentalcare.com  and other online resources,  a research paper, a  power point presentation over the research paper,  and a comprehensive final examination.

2.  Literature Review, Research Paper and Power Point Presentation

A research paper and oral report based upon a review of the current literature on a topic of your choice will be required for this course.   The topic must be related to the "special needs" patient and include modifications that may be required in   dental hygiene care and treatment planning.  The research paper requires a minimum of six current scholarly journal references and must have at least eight typed pages utilizing the current APA format.  The eight page minimum does not include the cover page or the works cited  page.  Students will be required to turn in copies of the research articles along with the research paper. 

An oral presentation of the research paper  in the form of a Power Point presentation will also be presented to your class and to the dental hygiene faculty.    The presentation times will be scheduled on two different Friday afternoons from approximately 12:30 to 5:00  and ALL students will be required to come to class on these afternoons to  receive credit for this assignment.   Please see your syllabus calendar for the dates.    Each presentation should be approximately 15 minutes in length.  Please review the information given to you by the AC librarian before beginning your research paper.    Pay close attention on how to format a research paper in APA format including  references.    Students must meet with staff at  the AC Writers' Corner for approval of the first draft.  Please hand in documentation showing your time and name of the  instructor who worked with you at the AC Writers’ Corner.

Both the research paper and PowerPoint presentation will be turned in for grading and then will be placed in the student's file.  If you would like, please keep a copy for your files. The oral presentation will also be evaluated by your peers.  Place the following items in a folder to turn in for grading:  research paper, articles, and a  flash drive with your  PowerPoint presentation saved on it.   If you would prefer to provide an electronic copy of your research articles on your flash drive you may do so.   Your folder should have your name, date, class and year clearly labeled on the outside front cover.  Rubrics for grading  are provided in Blackboard for both the Research Paper and the Power Point Assignment.

4.  Final Examination

The final examination will be comprehensive in nature over all of the material covered during the semester.

The final course grade will be computed as follows:

The following course grade scale applies throughout this course:

A = 93-100

B = 83-92

C = 75-82

F = Below 75%

Regular attendance is necessary for satisfactory acheivement.  Therefore it is the responsibility of the student to attend class.  Due to the tremendous amount of information contained in this course, the student who plans to succeed should also plan to attend all course sessions regularly and promptly.  Without question, the instructor expects students to be present at each session.  Unfortunately, no one has ever developed a short cut which will replace hours of actual experience needed to master a new skill; therefore, you must be present to acquire the specific knowledge in this subject.  Reading assignments and reviewing the power point associated with the material must be done prior to class.

Absences will be monitored and evaluated to determine the student's final course grade. The student may have no more than 1 absence in this course without affecting the final grade.  Beginning with the 2nd absence, 2 points will be deducted from the final grade; on the 3rd absence, 3 points will be deducted; on the 4th absence, 4 points will be deducted.  On the occurrence of the 5th absence, the student will fail the course and be required to repeat the course.  Attendance will be taken at the beginning of each lecture and lab session.  Students who are  not present when attendance is taken will be counted as absent.   STUDENTS WHO ARE LATE TO CLASS OR LAB OR WHO LEAVE CLASS OR LAB EARLY WILL BE COUNTED AS ABSENT.  The student will receive 2 bonus points to the final grade for  this course if the student has perfect attendance for the semester.

Make-up Policy

If a student is absent on the day when a major examination is given, the student may make-up the missed work as follows:

1.  It is the responsibility of the student to make arrangements with the instructor within 24 hours of returning from the absence to reschedule the examination.

2.  The missed work must be made up within TWO Amarillo College school days where the day ends at 4:00 p.m.

3.  The makeup work may earn a maximum of 80% of the original point value.
