1. Complete the Disability Services Application Form

• You can access the Disability Services Application Form through the AC Connect Self Service portal, under AC Forms — Disability Services.

2. Submit Documentation

• Most recent 504 plan or IEP (ARD) document AND
• FIE (Full Individual Evaluation) or medical documentation
  Note: Your school counselor may be able to help you obtain this documentation

3. Participate in an Intake Appointment

• Disability Services Staff must meet with you.
  • We cannot meet with your parent/guardian, teacher, counselor, or anyone else. We must meet with you.
  • It is your responsibility to schedule this appointment with our office.
  • If you would like someone to come to this appointment with you, you may ask them to, but you will be required to speak for yourself.
  • We have office hours to accommodate after-school appointments, but if your work or extracurricular commitments do not allow for in-person appointments, virtual appointments may be scheduled on a case-by-case basis.

An accommodation plan is a list of approved accommodations, determined on an individualized basis, with the purpose of providing access to the environment, program, course, etc.

Disability Services staff will review all information to create an individualized accommodation plan that addresses your specific needs.

• Accommodations are determined based on college-level considerations and criteria and are not likely to remain the same as high school accommodations.

If you are approved for an AC Accommodation Plan, you will receive an email (to your Amarillo College student email address) with instructions for accessing your accommodations.

• Accommodations must be emailed in PDF form to the instructor for each Dual Credit course you are enrolled.

Accommodations are determined based on the individual needs of the student. Some possible accommodations may include:

• Alternative formats (Braille, e-text, large-print, text-to-speech)
• Alternate furniture Alternate testing environments
• Extended testing time
• Note-takers
• Note-taking software or recorders
• Sign Language Interpreters

While accommodations may be provided in high school, and some of those accommodations may look similar in the college setting, there are many differences between the services provided in these two settings. See the chart below for more information.
